The Resident and Family Navigator coordinates and delivers appropriate support to residents and their families as needed and serves as the primary contact for families.  This role helps to ensure a smooth and seamless transition into the community and ongoing.  The Resident & Family Navigator also acts as a referral source directing participants and their families to community social services providers when appropriate.

Responsibilities:

  • Meets and greets new resident/family to our community
  • Supports wellness needs of our resident by ordering any necessary resident equipment related to meeting wellness needs
  • Ensure smooth move in for resident and family member, answers pre-move in questions, ensures all paperwork is completed and all necessary pre-move-in activities have been completed. 
  • Supports family members with resident’s admission, changes in condition of physical and emotional health, and discharge
  • Participates in family council meetings and actively pursues and addresses concerns that have been raised in a timely manner.
  • Facilitates Family Support Group for families & friends of the community.
  • Meets residents and family members privately upon request
  • Actively participates in the case management process
  • Assists the Community Sales Team with sales and marketing programs and initiatives

 

Job Requirements

Requirements:

  • Licensed Social Worker is preferred 
  • Bachelors Degree in Healthcare or related field 
  • Senior Living experience required, and Mind and Memory Care experience preferred.
  • Certified Dementia Practitioner
  • Dementia/elder care experience
