Job Description

 

PURPOSE

Benchmark Senior Living is in search of a Social Worker at our community. The Social Worker will coordinate and deliver appropriate social services to residents and their families as needed.  You will be a referral source for directing participants and their families to community social services providers when appropriate. The Social Worker will also facilitate the GAFC program within the community. This position reports to the Executive Director at the community they are assigned to. 

 

Responsibilities:

  • Organizes and manages the GAFC service program to meet the needs of residents
  • Coordinates social service needs for residents
  • Participates in screening, assessment and reassessment interviews of existing and potential residents
  • Participates in the resident’s service plan development
  • Participates with community team in move-in and discharge/move out planning activities as applicable
  • Supports family members with resident’s adjustment, changes in condition of physical and emotional health, and relocation
  • Participates in resident and family council meetings/support groups/educational offerings upon request
  • Meets with residents and family members privately upon request
  • Completes documentation in accordance to State regulations and BAL standards/practices including but not limited to: GAFC program, Low Income housing program, Resident service needs, incidents, associate relations, community incidents
  • Any/all responsibilities as assigned by the Executive Director

 

 

 

 

Job Requirements

EDUCATION/SKILLS

  • At minimum a valid license in social work
  • A bachelor’s degree in social work 
  • Proficient in using computer based technology
