Our community in Smithtown, is seeking a compassionate and caring Senior Director of Nursing to oversee the successful operation of the resident care department of two or more communities. In addition to fulfilling the Director of Nursing responsibilities at your primary community, the Senior Director of Nursing is responsible for implementing, teaching, and ongoing compliance monitoring of new initiatives, Benchmark policies and government regulations at one or more additional communities under their scope of supervision.

The Senior Director of Nursing will regularly update the regional team on resident care status of community that the Senior Director of Nursing supports.This role will also serve as the liaison with family members, community agencies, physicians, and outside care providers.

Job Responsibilities:

  • Identifies and assesses the clinical status of potential residents. Obtains the medical information release form from primary care physician as needed.
  • Develops an ongoing plan of care based on the identified physical and psychological problems of the resident including the identified problems; resident specific goals and planned program interventions.
  • Develops a service plan according to the requirements outlined in the state regulations.
  • Readily responds to care questions from RCDs in assigned communities.
  • Effectively balances service demands while supporting resident independence and choice.
  • Accurately assesses, communicates, and documents residents' status.
  • Observes and reports any significant changes in resident behavior and health to the RCD, physician, family, RCAs, and Executive Director per state regulations.
  • Successfully implements the residents ‘plan of care.
  • Recognizes and assists in the prevention of elder abuse, neglect, and exploitation (financial and other) and reports to appropriate sources per state regulations.
  • Effectively communicates with families and friends of residents.
  • Efficiently documents and provides accurate record-keeping as appropriate.
  • Effectively communicates with other care providers in a professional manner.
  • Assures Resident Information is communicated to the staff through a report process and calendar (appointments, outings, etc.).
  • Accurately completes incident reports prior to the end of the working day.
  • Notifies regional team of significant incidents in a timely manner.
  • Effectively handles follow-up issues with families.

Job Requirements

Job Requirements:

  • Being a Registered Nurse (RN) 
  • Must have a minimum of two years of experience working as a Director  as well as demonstrated supervisory and management skills.
  • Valid driver's license.
  • CPR certification required or ability to obtain within short timeframe of hire date.
  • Minimum 2 years of experience in a care setting; Assisted Living/Home Care or Hospice setting experience a plus
  • Knowledge of Dementia care a plus
