Signature Services Associate/ Licensed Nursing Assistant

We are looking for compassionate Signature Services Associates - to join our growing team of assisted living professionals at Benchmark Senior Living. Signature Services is a unique position here at Benchmark-it is a companion service to our residents that may simply enjoy the company, require it for safety, require assistance to go to a doctor's appointment, shopping, on trips with the community or even to a family function.

The Signature Service Resident Care Associate provides the service and companionship required to each resident in a timely and efficient manner as outlined on the service plan. The Signature Service Associate collaborates and works with the community resident care staff in order to consistently establish this cohesive delivery of care to the resident.

Job Requirements


  • Effectively communicates with residents, families and friends of residents
  • Efficiently documents and provides accurate record-keeping as appropriate
  • Assures resident information is communicated to the staff through the report process and calendar (i.e., appointments, outings, etc.)
  • Communicates and documents residents’ material needs (i.e., briefs, clothing, toothpaste, pads, etc.)
  • Accurately completes incident reports prior to the end of the working day
  • Appropriately handles follow-up issues with families

Customer Service

  • Supports the resident, family, friends and others as a member of the community resident care team. Works with the community team to establish cohesive service delivery for the resident.
  • Demonstrates understanding of the emotional stress of having an aging loved one by responding professionally and sensitively to the requests of family.
  • Responds to resident / family requests in a timely manner
  • Follows through with resident and family needs by communicating with the TCD- HCD- RCD.
  • Meets customer service standards in a friendly, helpful, courteous manner and makes customer satisfaction a top priority

Resident Care-General

  • Assists assigned resident with Activities of Daily Living
  • Participates in service plan reviews where requested
  • Removes, launders, folds, and properly returns resident linens and personal laundry
  • Serves resident food as directed, including feeding skills, if regulations permit
  • Knows and complies with all resident rights
  • Receives and gives report to incoming shift by oral and/or written log
  • Informs supervisor of changes in the resident’s condition and needs
  • Successfully implements the plan of care
  • Recognizes and can assist in the prevention of elder abuse, neglect, and exploitation (financial and other)
  • Reacts calmly and effectively in emergency situations

Resident Personal Hygiene & Well being

  • Checks resident daily for cleanliness, appropriate clothing, and any continence issues
  • Communicates and documents residents’ material needs (i.e. briefs, clothing, toothpaste, pads, etc.)
  • Effectively balances service demands with supporting resident independence
  • Accurately checks, communicates, and documents resident status
  • Observes and reports any significant changes in resident behavior and health
  • Provides ongoing supervision of residents health related activities: complies with health care instructions explained by health care professionals and promptly and appropriately seeks and obtains medical attention when needed
  • Responds to emergencies and personal emergency response system and knows when to call for backup


  • Valid LNA license required


  • Demonstrates knowledge of the following Benchmark Standards and Practices and equipment:
  • OSHA guidelines, e.g., chemicals, personal care products, and items locked
  • Use of walkie talkies and keys
  • Nametag and uniform
  • Proper punching of timecards
  • Alarm system
  • Fire safety and elopement
  • Consistent attendance and participation in staff meetings and mandatory in-services where requested
  • All equipment in resident’s room
